Alternative Placement Request Form

Note: A separate application is required for each student.

Statute Notice:

Any Unorganized Territories eligible resident secondary student may attend as a tuition student any public or private secondary school approved for tuition purposes under section 2951 to which that tuition student may gain entrance. Tuition students shall be entitled to all privileges and benefits, and shall be subject to the same rules as resident students of a receiving school administrative unit or other enrolled students of a receiving private school. The EUT will designate a high school for EUT students to attend within their region, but parents may apply for admission, enroll, and designate another secondary school for their student to attend. If accepted in the enrolling school, the EUT shall pay tuition for each tuition student in an amount equal to the official EUT secondary tuition rate approved pursuant to section 7302 for the receiving school for nonresident students as long as the student maintains a satisfactory standard of deportment and scholarship. §3253-A.
The EUT has assigned high schools for every unorganized township. Transportation to designated secondary schools may be paid in full or in part by the commissioner except that the commissioner may not reimburse parents for the transportation of a student. If a secondary student elects to attend and is accepted to enroll in a non-designated secondary school the EUT may assist in seeking alternative transportation. Parents may be required to transport secondary school students who attend a non-designated secondary school.

Acknowledgement Statement

1. This form is required for any student in grades Pre Kindergarten to Grade 8 seeking an Alternative Placement or eligible resident secondary students seeking to attend any public or private secondary school approved for tuition purposes.

2. Parents/Guardians must be aware that if the request for an Alternative Placement is approved, tuition for the Alternative Placement is directly connected to the primary residence of the student. It is further understood that tuition costs will become the parent or the attending district's responsibility should the primary residence change to a residence outside of the Unorganized Territories during the school year.

3. If approved, transportation to the alternative placement will become the responsibility of the parent or guardian.

4. Students Entering Grade nine through 12 may attend any high school that they can gain admission. The EUT will provide transportation only to the high school designated by the Education in the Unorganized Territories

Proof of Residence

Proof of residence must be attached to this form, emailed or mailed to the EUT Office in Augusta before a decision can be made on this application. You may send an electronic document or photo evidence of residence to EUTADMIN@maine.gov or mail it to EUT, #136 State House Station, Augusta, ME 04333-0136 Proof of residence may include one of the following: a tax bill, utility bill, lease agreement, mortgage or bank statement.
